Family and household coverage illustration

Individual

Coverage for the named member. Choose this option when you are enrolling only yourself.

Family

You, your spouse, and your dependent children under age 26. List each covered person’s first name, last name, and date of birth.

Household

Everyone included in Family, plus up to three additional individuals who live full time at your address. Add their names and dates of birth to the membership roster.

A person’s eligibility is defined by the Membership Agreement; being at the same address does not automatically add them to a membership.

BEFORE YOU CHOOSE

A few practical things to consider.

Review your health plan

Check both ground and air ambulance benefits. Look at deductibles, copays, coinsurance, and any other coverage you already have.

Choose your billing schedule

Monthly plans spread payments across the year. Annual prices are set separately for each product and coverage type. Compare the annual total with 12 monthly payments; any savings or additional cost appears beside your selected plan.

Read the membership terms

Confirm benefits, exclusions, state eligibility, and claim requirements. The Membership Agreement—not a website summary—governs your membership.

These plans focus on emergency ambulance transport

They do not include elective recovery transfers, visitor travel, home-return travel, vehicle or pet return, organ transport, or other travel assistance benefits. An emergency membership does not replace health insurance or an ambulance service.
